As a service, Pasco County Schools provides a group insurance plan(policy)through Bollinger that is underwritten by Mutual of Omaha Insurance Company to assist families with some of the medical expenses that may result from a school sponsored and supervised activity. This coverage is available for all of our students/athletes and is subject to policy conditions, exclusions and maximums. This plan pays in excess of any in force insurance plan for the student and does not cover students if injured outside of school

• If a registered student is injured during school hours or a sponsored event, the school completes the student accident report and submits to Risk Management.

• Based on the information listed on the report received, Risk Management will send a letter and claim form to parents(Guardian) to be completed.
• The group plan covers all enrolled students at no cost for school sponsored and supervised activities

A 24-hour voluntary plan can be purchased by parents(Guardian) and covers injuries incurred 24 hours a day at home, school, etc.

• Parents(Guardian) can access the form, select the 24 hour coverage option, and mail it to the carrier.
