Related Services Evaluation Procedures


Related Services: Assistive Technology (AT)

  • The 504 Team should gather school-based evaluation data to determine appropriate accommodations (e.g., word processing, dictation, etc.)
  • Schools have various staff members who can become part of the 504 Team when considering assistive technology. Individuals may include AT/CORE Liaisons, ESE teachers, Occupational Therapists, technology specialists, etc. Please consider the needs and expertise of the individuals on each school campus. This individual does not need to be knowledgeable concerning each type of assistive technology, but rather, assist the team in collecting data to determine effectiveness of accommodations including assistive technology.
  • If school-based team is unable to make recommendations based on existing data,or additional data collected after expanding the team to include the person listed above, the 504 Coordinator can contact the AT/CORE team for consultation by emailing Kim Taylor at kataylor@pasco.k12.fl.us

  • Once the AT/CORE team consults with the team, there may be a recommendation for further evaluation based on the input from the AT/CORE team and school based team. If so, the following procedures are required:
  • 504 Consent for Reevaluation is completed and sent with a AT/CORE Initial Referral form to Kim Taylor at Charles S. Rushe Middle school. Form located at: AT/CORE Team Referrals
  • The AT/CORE team notifies the 504 Coordinator of the evaluation completion
  • A 504 Re-evaluation meeting is held to discuss the results of the evaluation and recommendations for adding 504 Accommodations, if applicable
  • Two copies of the evaluation are made; one copy for the 504 folder, and one copy provided to the parent or guardian


  • If technology is recommended that the school or district must purchase, please notify your school-based administrator who will need to follow up with the District-level ADA Coordinator (Employee Relations Director).

Related Services: FM System Requests

Consideration of requests for an FM system will be made for students who present:

(1) significant hearing loss based on recent Audiological evaluation, or
(2) poor speech discrimination in noise based on a recent Auditory Processing Disorder.


Upon request for an FM system, an observation of the student will be conducted in the classroom environment with the purpose of determining the listening demands for the student and to identify reasonable accommodations. Reasonable accommodations for students with disabilities are required by law, but may not necessarily result in a recommendation for FM system if other reasonable accommodations provide adequate access to the educational environment. If an FM system is recommended, the type and style of system shall be determined by the district audiologist based on age of student, type of hearing loss, and classroom dynamics.


Related Services: Occupational, Physical, Speech/Language Therapies

  • 504 Consent for Reevaluation is required if considering related services on a 504 Accommodation Plan
  • For Occupational Therapy (OT), Physical Therapy (PT), Speech or Language Therapy (Sp/L), submit a screening form and submit it to your school's assigned therapist
  • For OT or PT: An email must also be sent to the respective therapist and Glennda McCallister (gmccalli@pasco.k12.fl.us) indicating a request for a screening
  • The therapist observes the student and can recommend
  • Classroom-based accommodations
  • No accommodations are necessary
  • Pursue a more formal re-evaluation, and may request a prescription from the student's physician (specifically for OT & PT)
  • If a full evaluation is conducted by the therapist:
  • The therapist notifies the 504 Coordinator of the evaluation completion
  • If a related service is recommended by OT or PT, a peer review at District Office is initiated by send a copy of the existing 504 folder (with the current 504 plan pre-evaluation) and re-evaluation results to Glennda McCallister who will contact the 504 Coordinator re:approval of related service for 504 Accommodation Plan
  • A 504 Eligibility or Re-evaluation meeting is held to discuss the results of the evaluation and recommendations for adding 504 Accommodations, if applicable
  • Two copies of the therapist's evaluation are made; one copy for the 504 folder, and one copy provided to the parent or guardian
