Continuity of Care is a commitment to reach individual goals by building ongoing, supportive, nurturing relationships in an environment that responds to individual needs.
Continuity of Care is reflected in school programs that meet the full range of academic and developmental needs of all students throughout the prekindergarten through adult educational experience. Continuity of Care demonstrates that students are valued. Decisions in the classroom, in the school, and in the district are made based on this value that considers the whole child — his/her academic, social, emotional, and physical needs.
