How to Obtain a Voluntary Pre-K Certificate of Eligibility

Items needed in order to receive a Certificate of Eligibility (COE):
• Child’s proof of age (i.e. original birth certificate, original shot record, or passport)
• Proof of residency (i.e. current driver’s license or utility bill) dated within 3 months
Parents can apply in person at one of the locations below or online at http://www.phelc.org.
The link below will take an applicant right to the online application for the certificate. The parent will be required to upload the documents above as well.

Parents who obtain a COE must email Alexis Russell at .(JavaScript must be enabled to view this email address) in regard to their interest in the program, their workplace location and the school that they are choosing for their child.
