Drug and Alcohol Testing
On September 1, 2008, the District School Board of Pasco County implemented a Reasonable Suspicion Drug Testing Program. The forms pertinent to enforcing the program are available below. They consist of:
- Drug and Alcohol Awareness Manual (en español)
- Companion Video
- Acknowledgment Form: to be signed by each new employee after they have received the Drug and Alcohol Awareness information booklet;
- Drug Testing Program Notification Form: to be completed by the trained supervisor and taken to the collection technician; and,
- Supervisor’s Observations Form: to be completed by the trained supervisor who observed and recorded the observation of the employee under suspicion.
- Approved Drug/Alcohol Collection Sites Form: These are the only sites you should consider for collection of specimens of suspected offenders.
In addition to the forms listed above, Florida Drug Free Workplace Chain of Custody Forms are provided to each worksite and additional copies may be obtained by calling the Department of Employee Relations at extension 42018.
