Report a Death

We wish to extend our sincere sympathy.

This form may be used to report the death of a Pasco County Schools employee, retiree, or covered dependent.  In order to report a death online, we will need the following information about the deceased person:

  • Name
  • Date of Death
  • Employee ID Number or Last Four Digits of SSN
  • Information concerning who to contact regarding the deceased person.

You will recevie an email from riskmanagement@pasco.k12.fl.us within 24 hours confirming receipt of your online report.  You will also receive beneficary claim forms from Minnesota Life/Securian.  Please complete the forms and return directly to Minnesota Life/Securian for processing.
